Volunteers and staff are required to fill out, sign, and date the listed forms. If you are a parent that helps out, you should also complete these forms. Volunteers under the age of 18 will need a parent or guardian to sign their forms.

 

These forms must be reviewed annually and updated if necessary. 

 

In the event you are injured and are taken to the Emergency Room, Crossroads Center will provide the hospital with your medical information.
